Community First Services and Supports (CFSS) is an innovative self-directed program designed by the state of Minnesota to offer more choice, control, and flexibility for individuals who require home care services. We are proud to provide CFSS services through the agency model, empowering clients to live fulfilling lives in their communities.
What is CFSS?
CFSS aims to replace the current Personal Care Assistance (PCA) program and the state-funded Consumer Support Grant (CSG) benefit, offering assistance with Activities of Daily Living (ADLs), Instrumental Activities of Daily Living (IADLs), health-related tasks, and more.
The program provides increased independence, the ability to purchase goods and services to aid in clients’ autonomy, and options for employing family members, including spouses and parents of minors, as support workers.

We offer the agency model of CFSS, ensuring our clients receive high-quality care and support. In the agency provider model, we will recruit, hire, train, supervise, and pay CFSS support workers, while clients direct their care and choose their workers.

CFSS is a Minnesota healthcare program designed to offer more choice, control, and flexibility for individuals requiring home care services. It aims to replace the Personal Care Assistance (PCA) program and the Consumer Support Grant (CSG) benefit.
CFSS covers assistance with Activities of Daily Living (ADLs), Instrumental Activities of Daily Living (IADLs), health-related tasks, observation and redirection of behavior, and more.
In the CFSS agency model, Safeway Home Healthcare serves as the employer of the support workers, handling recruitment, hiring, training, supervision, and payment, while the clients direct their care and choose their workers.
Safeway Home Healthcare currently offers only the agency model of CFSS to provide high-quality care and support for clients.
Clients will transition to CFSS at the time of their assessment. Safeway Home Healthcare will provide guidance and support during this transition to ensure a seamless experience.
The eligibility requirements for CFSS are similar to those for PCA. A person must live in the community, be able to direct care or have a representative to do so on their behalf, be enrolled in one of Minnesota’s healthcare programs, have an assessment that determines their eligibility, and not be using the Family Support Grant.
Yes, under CFSS, a person’s spouse or the parent of a minor may serve as that person’s support worker.
In the CFSS agency model offered by Safeway Home Healthcare, clients will be able to keep their current trusted caregivers.

The implementation of CFSS is subject to federal approval. Once approved, the Minnesota Department of Human Services (DHS) will provide information about the transition plans and time frame.
