The Referral Process: Eligibility and Assessment

To receive services, clients must meet certain eligibility requirements. Safeway Home Health Care conducts an initial assessment to determine the client’s needs and develop a personalized care plan. Getting Started with Referrals

The case manager makes a referral to the agency, either by phone or through the online referral form available on the website.

Intake Scheduling

Upon referral, we schedule an intake meeting within 10 working days. This is a pivotal step in getting started, offering options for in-person, phone, or video conferencing meetings.

The intake meeting involves signing admission paperwork and understanding our policies. It’s also a chance for clients to inquire about getting started with us.

Support Plan Development

After the intake meeting, we develop a support plan tailored to each client’s unique needs and goals. This plan is shared with the client and their care team, a key part of getting started with Safeway Home Healthcare.

Caregiver Matching

Within three weeks of the intake meeting, the client will be matched with a caregiver who is best suited to meet their needs and goals. The agency will work to ensure that the client is comfortable with their caregiver and that the caregiver has the necessary skills and experience to provide high-quality care, a critical step in getting started with our care process.

Quality Assurance Reviews

Reviews Regular quality assurance reviews will be conducted on an ongoing basis to ensure that the client is receiving high-quality care and that their support plan is being implemented effectively. We will work closely with the client and their care team to identify areas for improvement and make adjustments as needed. This is an integral part of getting started with ongoing care.
